The short answer
File Form SSA-44 to ask Social Security to use a more recent, lower income year after one of eight life-changing events, including work stoppage at retirement. The 2027 surcharge is based on 2025 MAGI. In 2026, IRMAA started above $109,000 single and $218,000 joint and added $81.20 to $487.00 to the $202.90 Part B premium. CMS sets the 2027 figures in the fall.
- Qualifying events: Eight only: death of spouse, marriage, divorce or annulment, work reduction, work stoppage, loss of income-producing property, loss of employer pension, employer settlement payment
- Tax year used: 2027 IRMAA comes from your 2025 return; 2026 IRMAA came from 2024
- 2026 thresholds: Above $109,000 single or $218,000 joint, adding $81.20 to $487.00 to the $202.90 standard Part B premium
- 2027 status: CMS has not announced the 2027 income thresholds and amounts; the 2026 figures were released November 14, 2025
Applies to people with Medicare Part B or Part D who received an IRMAA determination notice and whose income fell because of one of SSA's eight listed events; the form is not for income drops with no listed cause.
What counts as a life-changing event for IRMAA?
Social Security recognizes exactly eight: death of a spouse, marriage, divorce or annulment, work reduction, work stoppage, loss of income-producing property, loss of employer pension, and a settlement payment from a current or former employer. The list is closed, so an income drop with no listed cause does not qualify.
Retirement is the common one. Stopping work is a work stoppage, and cutting back hours is a work reduction. Both have their own instruction sections in Social Security’s manual.
The event must cause a significant reduction in modified adjusted gross income, meaning a reduction that decreases or eliminates the income-related monthly adjustment amount for a specific tax year. Social Security does not develop the types of income that make up MAGI, only that MAGI decreased and that the event happened before the decrease.
Events that raise your costs do not count. The manual lists ordinary loss of dividend income, higher medical expenses, higher living expenses, loss of child support, loss of alimony, and a voluntary sale of income-producing property as non-qualifying. How do you file Form SSA-44 for 2027?
You can sign in at SSA.gov and upload the form, fax or mail it with evidence to a Social Security office, or create an account and schedule an appointment. The form runs five steps: the event and its date, the reduced-income tax year, an optional second-year estimate, documentation, and your signature.
Print your name and your own Social Security number as they appear on your Social Security card, which may differ from the number on your Medicare card. Step 1 asks you to check the life-changing event and fill in the date it occurred.
Timing matters more than people expect. The event date must be in the same year as, or an earlier year than, the tax year you ask Social Security to use. If you have not filed a return for the more recent year, you supply an estimate, and you may give a second estimate for the next premium year if the event affects that year too.
Step 2 wants your adjusted gross income plus tax-exempt interest and your filing status for that year. People who are brand new to Part B often hit IRMAA in month one; our 7-month enrollment checklist shows where the premium notice lands in that sequence.
What documents does Social Security require as proof?
Two kinds: proof that the event happened, and proof of the income you are claiming. You either attach the required evidence, and SSA mails your original documents or certified copies back, or you show originals or certified copies to an SSA employee.
Match the document to the event. Useful items include your applicable 1040 tax returns and evidence of the life-changing event, such as an official marriage certificate, a divorce decree, or an employer separation letter. A pension termination letter or settlement agreement works the same way for those events.
The form notes that you must sign in Step 5, attach all required evidence, and provide a current address and phone number so SSA can reach you with questions. An unsigned form or a missing proof document is the most common reason a request stalls.
How long does an IRMAA appeal take, and can you get a refund?
Social Security does not publish a standard processing time. For a current-year event, the manual sets the effective date retroactively: the new initial determination for Part B is effective January, or the first month of Part B entitlement, of the current year.
That retroactive date is what produces money back. If a higher premium was already withheld from your Social Security benefit for those months, the corrected determination applies to them.
Drug coverage follows its own clock. For drug coverage IRMAA, if your plan was effective for the entire year of the request, the new determination is effective January of that year; otherwise it starts on the coverage start date that year. If your plan premium is also moving, read our breakdown of 2027 Part D premium increases alongside the surcharge, and the Part D hub for how the two stack.
What if your income dropped but no listed event caused it?
Then SSA-44 is the wrong form. Higher medical expenses, higher living expenses, lost alimony or child support, ordinary loss of dividend income, and a voluntary sale of income-producing property are listed as non-qualifying. Amended returns run on a separate track by phone.
A one-time income spike is the hard case. A Roth conversion, an exercised stock option, or a large capital gain in 2025 can push your 2027 premium up, and none of those is on the list of eight. The surcharge then falls off on its own once a lower tax year cycles through, which is two years later.
For an amended income tax return, SSA says to call 1-800-772-1213 and tell the representative you want to lower your IRMAA. Separate manual sections also cover corrected IRS data, two-year-old data when SSA used three-year-old data, and married filing separately while living apart all year.
How does this interact with the 2027 IRMAA brackets?
The 2027 surcharge comes from your 2025 modified adjusted gross income. CMS has not yet announced the 2027 income thresholds and amounts, and the 2026 numbers were released on November 14, 2025. Your 2027 determination notice arrives after that release.
| Item | 2026, confirmed by CMS | 2027, as of October 23, 2026 |
|---|---|---|
| Tax year used | 2024 return | 2025 return |
| Standard Part B premium | $202.90 a month | Not announced; Trustees project $209.50 |
| First IRMAA threshold | Above $109,000 single, $218,000 joint | Not announced |
| Part B IRMAA add-on | $81.20 to $487.00 a month | Not announced |
Treat the 2027 column as pending, not as a rate. Once CMS publishes, run your 2025 figures through our IRMAA calculator to see which tier your return lands in, then decide whether an SSA-44 is worth filing.
One more sequencing point. The form is meant for people who received a notice that their Part B or drug coverage premiums include an IRMAA, so the notice is usually your starting document. Other Medicare questions in the same season are covered across our Medicare hub.
Your SSA-44 checklist
- Find your IRMAA determination notice and note which tax year Social Security used.
- Match your situation to one of the eight listed events and write down the month and year it happened.
- Pull the proof: the event document in original or certified-copy form, plus a signed return, transcript, or a written estimate.
- Complete Steps 1 through 5 of the current SSA-44 edition, including the signature.
- Upload it after signing in at SSA.gov, or fax or mail it with evidence to a Social Security office.
- Keep copies, and re-check the determination after CMS publishes the 2027 premium and thresholds.
Questions people ask
Does Form SSA-44 cover both Part B and Part D?
Yes. SSA says a life-changing event that reduced household income lets you ask to lower the additional amount you pay for Medicare Part B and Part D. One approved request adjusts both surcharges, because both are driven by the same modified adjusted gross income figure.
Should I file if my last tax return was under the threshold?
No. The form is for people who received a notice that Part B or drug coverage premiums include an IRMAA, and SSA's instructions say you should not complete it if your MAGI was below the filing threshold, even after a life-changing event.
What if two life-changing events happened?
Call Social Security before filing. SSA's support line is 1-800-772-1213, and you tell the representative you want to lower your IRMAA due to a life-changing event. Wording on multiple events has changed between form editions, so confirm with the current edition.
Is an amended tax return handled on SSA-44?
No. SSA directs you to call 1-800-772-1213 and tell the representative you want to lower your IRMAA if you had an amended income tax return. That request follows a separate manual section from the eight life-changing events.
Where does the completed form go?
You can sign in at SSA.gov and upload it, fax or mail the form and evidence to a Social Security office, or create an account and schedule an appointment. Keep a copy of everything you send.
