Illinois TRAIL Medicare Advantage Deadline Is November 23, 2026, Not December 7

State, teacher, and community college retirees on TRAIL face a shorter window than other Medicare beneficiaries, and a BJC HealthCare-Aetna contract dispute makes this year's deadline matter more than usual.

The short answer

Illinois TRAIL retirees must make 2027 Medicare Advantage elections between October 15 and November 23, 2026, not by the standard December 7 AEP deadline. Illinois CMS confirms no elections are accepted before or after that window. The deadline matters more this year because BJC HealthCare is dropping Aetna Medicare Advantage, affecting about 5,000 retirees who use Barnes-Jewish Hospital and WashU Medicine.

  • TRAIL 2027 Open Enrollment: October 15 to November 23, 2026, set by Illinois CMS, two weeks before standard AEP ends December 7.
  • BJC-Aetna network exit: Barnes-Jewish Hospital and WashU Medicine providers become out-of-network for Aetna Medicare Advantage on January 1, 2027.
  • Retirees affected: About 5,000 TRIP, SEGIP, and CIP retirees who recently used BJC or WashU Medicine providers, per Illinois CMS.
  • Covers: State Employees Group Insurance Program (SEGIP), Teachers' Retirement Insurance Program (TRIP), and College Insurance Program (CIP) retirees on TRAIL MAPD.

Applies to Illinois state, teacher, and community college retirees enrolled in TRAIL Medicare Advantage through SEGIP, TRIP, or CIP, especially those who use Barnes-Jewish Hospital or WashU Medicine providers.

What is TRAIL, and why does it have its own enrollment deadline?

TRAIL stands for Total Retiree Advantage Illinois, a group Medicare Advantage program run by the state, separate from the public Medicare AEP. It closes two weeks earlier than standard Medicare Advantage enrollment because Illinois CMS, not CMS federally, sets its own election window.

The State of Illinois offers retirees, annuitants, and survivors of the State Employees Group Insurance Program (SEGIP), the Teachers’ Retirement Insurance Program (TRIP), and the College Insurance Program (CIP) a healthcare program called Total Retiree Advantage Illinois, which provides Medicare-eligible members and their covered dependents medical and prescription drug coverage through TRAIL Medicare Advantage Prescription Drug plans. If you already receive retiree health coverage through one of these systems and you are enrolled in Medicare Parts A and B, TRAIL MAPD is almost certainly your current plan. For background on how Medicare Advantage works statewide, see our guide to Medicare Advantage plans in Illinois.

When exactly does the 2027 TRAIL open enrollment window close?

The 2027 TRAIL open enrollment period runs October 15 through November 23, 2026. Illinois CMS states plainly that no elections are allowed before October 15 or after November 23, with no exceptions for late requests.

The 2027 TRAIL Medicare Advantage Prescription Drug Open Enrollment Period runs October 15 through November 23, 2026. To make changes for the 2027 plan year, retirees must complete changes online at MyBenefits.illinois.gov or contact the MyBenefits service center during the Open Enrollment Period, and no elections are allowed before October 15 or after November 23. That is a full two weeks before the standard Medicare Annual Enrollment Period closes on December 7, which is the deadline most Medicare beneficiaries outside TRAIL are tracking. See our overview of AEP dates for 2027 coverage for how the general timeline compares.

What is the BJC-Aetna dispute, and who does it affect?

BJC HealthCare is ending its Aetna Medicare Advantage contract on January 1, 2027, making Barnes-Jewish Hospital and WashU Medicine providers out-of-network for TRAIL members who use them. Illinois CMS estimates about 5,000 retirees are affected.

BJC HealthCare announced in October that it is dropping its participation at the end of the year in the Medicare Advantage plans offered by Aetna and Anthem, which includes TRAIL, the state’s Aetna-administered Medicare Advantage program for eligible state, teacher, and community college retirees. The Illinois Department of Central Management Services says about 5,000 Illinois retirees who have recently accessed provider or hospital care will be affected when Barnes-Jewish Hospital and WashU Medicine stop accepting Aetna Medicare Advantage plans at the start of 2027.

Legislators from both parties have pushed back. A bipartisan group of state senators and representatives wrote that the loss of access to trusted healthcare providers would have significant consequences for affected retirees, who deserve continuity of care and certainty about their coverage. As of this writing, Aetna and BJC had not confirmed whether negotiations would resume before a new contract takes effect.

Can I keep seeing Barnes-Jewish Hospital or WashU Medicine doctors in 2027?

Only in limited cases tied to active treatment. BJC has agreed to honor continuity-of-care exceptions for medically necessary ongoing treatment, but routine visits and new patients will be treated as out-of-network once the contract ends.

Starting January 1, 2027, care received from BJC physicians, providers, and facilities will be considered out of network, and unless a specific out-of-network authorization is approved, such as for cancer treatment requiring continuity of care, standard out-of-network benefits apply. Patients may qualify for a continuity of care transition period for services that are part of an active treatment plan, or may receive authorization from Aetna for out-of-network services only BJC can provide in the region, but only Aetna can approve these exceptions.

If you are mid-treatment, ask your BJC care team and Aetna member services in writing whether your specific treatment qualifies, what it covers, and for how long. Do not assume a verbal assurance from either side will hold once billing starts.

What are my coverage options if I don’t want to stay on the Aetna TRAIL plan?

During the open enrollment window, you can request to disenroll from TRAIL MAPD and move to traditional Medicare with a separate Part D plan, or in some cases a different TRAIL-offered plan if your system provides one. Each option changes your network and your costs differently.

Moving to traditional Medicare does not automatically give you penalty-free, medically underwritten-free access to a Medigap policy. Federal guaranteed issue protections generally apply when your retiree plan ends entirely or your Medicare Advantage plan leaves your service area, not when a single hospital system like BJC exits a plan’s network while the plan itself keeps operating. Medicare beneficiaries have federally qualified guaranteed issue rights to purchase Medigap if their employer cancels their retiree coverage, but they do not have guaranteed issue rights if their retiree coverage simply changes or if they voluntarily drop it. That distinction matters here: TRAIL is not ending, only BJC’s participation in the Aetna network is.

Before assuming you have guaranteed issue rights, review the 10 Medigap guaranteed issue triggers and check how Illinois Medigap rules apply to your situation, since underwriting and guaranteed issue timing vary by state. Illinois also has its own annual birthday rule window separate from guaranteed issue; see how the Illinois 45-day birthday rule compares to federal 63-day rights if you are weighing a future switch. If you are unsure whether to stay on TRAIL or move to Original Medicare, our comparison of Medicare Advantage versus Medigap explains the coverage and cost tradeoffs.

What happens if I miss November 23?

If you miss the window, you keep whatever election is already on file, and you cannot add, drop, or change TRAIL coverage until the next annual TRAIL open enrollment. Illinois CMS states no elections are allowed after November 23. Waiting for the general December 7 AEP deadline will not help TRAIL retirees, since that deadline does not apply to this program.

For retirees in TRIP specifically, the stakes are higher than a simple plan change. TRIP members who are eligible for Medicare Parts A and B must enroll in the TRAIL Medicare Advantage plan to keep their TRIP insurance coverage after December 31. Missing the deadline without a valid enrollment on file can mean losing TRIP-sponsored coverage entirely, not just losing a plan choice.

Before November 23, 2026: a checklist for TRAIL retirees

  1. Confirm your personal deadline at MyBenefits.illinois.gov or by calling the MyBenefits service center, since some retirement systems have set slightly different internal dates.
  2. Ask your BJC or WashU Medicine provider’s office directly whether your ongoing treatment qualifies for a continuity-of-care exception, and get the answer in writing.
  3. List every provider and prescription you use, and verify each one’s 2027 network status before you decide to stay on TRAIL MAPD.
  4. If you are considering Original Medicare and a self-purchased Medigap policy, confirm whether you qualify for guaranteed issue rights or will face medical underwriting in Illinois.
  5. Submit any change, re-enrollment, or dependent update online or by phone before November 23, since no late elections are accepted.
  6. Keep a copy of your confirmation and any continuity-of-care approval letter for your records going into 2027.

Where can I get help deciding?

This decision involves provider access, prescription coverage, and potential Medigap underwriting questions that are specific to your health history and location. A licensed agent can review your TRAIL plan documents alongside Original Medicare and Medigap options for your situation. Questions people ask

Is TRAIL the same as regular Medicare Advantage AEP?

No. TRAIL is a separate group enrollment program run by Illinois CMS for state, teacher, and community college retirees. Its window closes November 23, 2026, two weeks before the standard AEP deadline of December 7.

What happens if I do nothing by November 23?

If you are currently enrolled in TRAIL MAPD and want to keep your current plan, you generally do not need to act. But if you want to change plans, drop coverage, or add a dependent, missing November 23 means waiting until the next TRAIL open enrollment.

Will Barnes-Jewish Hospital still see me under Aetna in 2027?

Only in limited cases. BJC has agreed to see patients when continuation of care is medically necessary, such as active cancer treatment, but routine and new care will be treated as out-of-network starting January 1, 2027.

Can I drop TRAIL and buy a Medigap policy instead?

You can request to disenroll from TRAIL MAPD during the open enrollment window and return to traditional Medicare. Whether you qualify for guaranteed issue Medigap rights without health questions depends on your specific circumstances and Illinois rules.

Does losing BJC as an in-network provider give me guaranteed issue rights?

Not automatically. Federal guaranteed issue rights generally require your retiree plan to end entirely or your Medicare Advantage plan to leave your area, not a single hospital system leaving the network while your plan continues.

Who do I call with TRAIL enrollment questions?

Contact the MyBenefits service center or make changes online at MyBenefits.illinois.gov during the open enrollment period, and confirm your personal deadline since some retirement systems set slightly different internal dates.
